The pattern I see
A patient comes in on their fourth antidepressant. Nothing has moved. Somewhere along the way someone added an anxiety label, then a bipolar question mark. By the time they reach me they've stopped believing anything will help.
Half the time the diagnosis was wrong from the start. Undiagnosed ADHD. Untreated trauma. Thyroid. Sleep apnea. A medication that was never dosed right. The other half, the diagnosis was right and the plan was lazy.
What I do differently
I start over. A full 60-minute evaluation, your history from the beginning, every medication you've tried and what happened on it.
I treat the whole picture. Medication when it helps. Sleep, hormones, nutrition, trauma history and daily structure when they're the actual problem.
I check in like it matters. 30-minute follow-ups by video, close together at first, then spaced out when you're stable.

Generalized anxiety, panic, social anxiety, major depression, treatment-resistant depression, PTSD and complex trauma, PMDD, mood disorders, and the overlap with ADHD that so often gets missed.
